Filovirus diseases, such as Ebola and Marburg, are severe and often fatal, with case fatality rates ranging from 25% to 90% during major outbreaks. Since the discovery of the Marburg virus in 1967, there have been 72 Ebola and Marburg disease outbreaks in Africa, significantly impacting affected communities socio-economically and psychologically.
WHO Director-General Dr. Tedros Adhanom Ghebreyesus highlights the importance of these guidelines, stating, "These new guidelines are a perfect example of how WHO leverages science to better protect and care for people during outbreaks and health emergencies." The guidelines are the result of global expert consultations and are based on the most current scientific evidence and clinical knowledge, translating lessons from recent outbreaks into practical recommendations for improved patient care. They aim to guide health workers in patient care, harmonize clinical approaches, and assist health facility administrators and policymakers in planning, preparing for, and responding to filovirus disease outbreaks.
Key recommendations include using prioritized clinical laboratory tests to monitor patients, quickly treating dehydration, promoting early and precise use of intravenous fluids and vasoactive medications for shock management, ensuring appropriate antibiotic treatment for bacterial infections, and providing structured after-care for survivors to prevent further infections. For Bundibugyo virus disease, early recognition, rapid referral, and optimized supportive care are crucial, reducing complications and serving as a foundation for clinical interventions and antiviral treatment research.
